I, hereby apply for enrollment as an athlete to PWSA (Home Court
Consulting LLC.) for the Fall 21 session in one of the following sports offerings; Baseball, Soccer, Basketball, and Tennis, from September 2021 to December 2021
Please refer to the housing application and housing contract for more details regarding the housing deposit.
The terms and conditions of said enrollment are set forth in this 2021 Payment Agreement Form (“the Agreement”) as follows:
1. I agree to pay Home Court Consulting LLC all applicable tuition of $11,500.00 (resident) or $9,800 (Commuter), fees, and charges, upon session start date, including:
A. Application Fee of $25.00. This fee will be charged to all new applicants and includes a transcript evaluation.
B. Initial $750 tuition deposit due immediately to secure your roster spot and applied to overall tuition cost.
C. 3% Processing fee added to every transaction when paying with a credit or debit card.
D. A $750.00 Refundable Housing Deposit due initially to ensure a limited housing spot.
E. A $200 Health Fee is due initially. (proof of medical insurance is required for participation)
F. A $100 furnished room Fee is due initially.
G. A $100 Utilities room Fee is due initially
Tuition Balance Payment Plan (after tuition deposit is applied): This agreement signifies that you agree with the following payment arrangement and terms:
Three installment payments:
Residential Commuter
1st payment $5,375.00 $4,615.00 within 1 month of tuition deposit paid
2nd payment $2,687.50 $2,307.50 within 1 month of 1st payment made
3rd payment $2,687.50 $2,307.50 before 2 weeks of session start date
All pending payments must be paid no later than September 1, 2021, in order to confirm your enrollment into the academy. Failure to comply may lead to loss of roster spot and refund/cancellation policy requirements. *Unless an accepted late enrollee after 8/1/21.
Refund/Cancellation Policies
Housing Deposit of $750.00 housing deposit is refunded at semester move out after the following room check out procedures and a signed Move-Out form with no damages to the facility or unit of residence.
Cancellations received less than 2 weeks before the session start date of enrollment is non-refundable
Cancellations received in advance of more than 2 weeks before the session start date of enrollment will receive a refund of tuition payments made at 50%.
Cancellations due to health or medical circumstances will be reviewed and decided on a case by case.
Any refund of tuition payments that are honored is minus the tuition deposit.
